What Is Liposuction?
Liposuction (lipoaspiration) is a plastic surgery procedure in which subcutaneous fat tissue is removed from targeted areas through specialised cannulas using suction. The technique has been refined continuously since the late 1970s and has since branched into several sub-techniques. The core principle is the same across all of them: a tumescent solution (saline + lidocaine + epinephrine) is infiltrated beneath the skin, the fat tissue is swollen and loosened by this fluid, and the fat is then aspirated through a cannula.
One important framework to establish at the outset: liposuction is not a weight-loss operation. It does not target the number on the scales. It reshapes body contours by reducing localised fat deposits that persist despite a stable weight. Fat cells are not regenerated in the treated area once they have been removed — however, remaining cells can enlarge with weight gain. The result is long-term, provided weight is managed.
Classic SAL, VASER, Laser (SlimLipo) — Technique Differences
The main techniques used at Nis Clinic:
- Classic SAL (Suction-Assisted Liposuction): Fat is aspirated through the mechanical movement of a cannula after tumescent infiltration. It is a proven, safe technique with the broadest evidence base, well suited to large-volume cases and anatomically straightforward areas.
- VASER (Vibration Amplification of Sound Energy at Resonance): Ultrasound waves emulsify the fat cells and separate them from surrounding tissues before aspiration. Because the fat is emulsified, the aspiration is more selective and trauma to blood vessels and nerves is reduced. It is preferred in fibrous areas (such as the male chest, back, and gynaecomastia) and where more detailed contouring is required. VASER also offers an advantage in fat transfer combinations (BBL, facial fat grafting, breast fat injection) because it preserves a higher proportion of viable cells.
- Laser liposuction (laser-assisted systems such as SlimLipo and SmartLipo): Laser energy liquefies fat while the heat effect can produce mild subdermal tightening. It is considered for small-volume areas with mild-to-moderate reduction in skin elasticity. It does not resolve skin laxity on its own.
There is no single "best technique". Selection is based on the anatomical area, skin quality, target volume and the desired level of definition. We practise technique selection per case — not a single-technique-fits-all approach.
360° Liposuction and Hi-Def (Athletic Etching)
Two additional concepts are commonly discussed in the world of liposuction:
- 360° liposuction: An approach in which the front, sides and back of the torso are contoured in the same session. It covers not only the abdomen but also the waist, flanks and upper and lower back — a circumferential plan. The result is a defined silhouette from every angle, not just the front. It is frequently combined with BBL, where the harvested fat is transferred to the buttocks to create a dual-direction contour improvement.
- Hi-Def (High Definition, athletic etching): An advanced technique in which VASER is used to accentuate muscle anatomy (rectus abdominis, obliques, pectoral borders) in the subcutaneous contour. It is suitable for patients who are already at a low body fat percentage and are aiming for an athletic appearance. This is not a standard procedure for the general population; candidate selection is meticulous and expectations are discussed in detail.
Both approaches are available at Nis Clinic — but we do not market Hi-Def for everyone or 360° for everyone. The technique appropriate for you is determined during consultation, following a body analysis and a goals discussion.
The Liposuction Process at Nis Clinic
Liposuction is not just a day in theatre. It is a journey that runs from candidacy assessment through to a six-month review. The final result settles 3–6 months after surgery, because the full resolution of swelling and tissue remodelling takes time. We approach the process in five stages.
Consultation — Candidacy Assessment and Skin Elasticity
We offer initial consultations online (Zoom or WhatsApp video call) or in person at the clinic. Most patients travelling from abroad prefer an online first appointment, followed by an in-person assessment before surgery.
During the consultation we discuss:
- Weight history: Has weight been stable over the past 6–12 months? Is a significant weight change planned in the near future?
- Map of concerns: Which areas bother you? Are the expectations realistic?
- Skin elasticity and presence of striae (stretch marks): Will the skin retract once fat is removed, or is there a risk of laxity?
- Previous surgeries, chronic conditions, current medications (particularly anticoagulants and hormonal treatments)
- Smoking: It has an adverse effect on wound healing; stopping at least four weeks before surgery is strongly advised.
- Diet and exercise habits: Will there be a sustainable routine to maintain results after surgery?
At the end of the consultation we give you a clear picture: whether you are a suitable candidate, which technique suits you, an approximate cost range, a recommended length of stay in Northern Cyprus, and what to expect during recovery. Telling a patient "this is not right for you" is, for us, as important as successfully operating on someone who is a suitable candidate.
Planning — Anatomical Mapping and Volume Estimation
Three steps take place at the in-person examination:
- Anatomical mapping: Areas to be treated are photographed whilst you are standing and marked with drawings. Symmetry, asymmetry and border zones are all planned.
- Volume estimation: The volume of fat to be removed from each area (in ml) is planned. The internationally accepted safety threshold for a single session is 5 litres (5,000 ml) of pure aspirate. Volumes above this threshold fall into the "large-volume liposuction" category and carry increased risks of fluid imbalance, electrolyte disturbance and pulmonary complications — requiring a different setting and additional precautions. Nis Clinic adheres honestly to this 5-litre threshold; claims of "10 litres removed in one session" have no safe clinical basis.
- Combination assessment: Can the harvested fat be transferred to the buttocks (BBL), used for breast fat injection, or will it be discarded? Where significant skin laxity exists, liposuction alone is insufficient — a combination with abdominoplasty (tummy tuck) or an alternative plan is discussed.
Planning determines most of the outcome. There is no "let the cannula wander" approach here. The operation begins on paper.
Procedure Day — Tumescent Solution, Anaesthesia and Flow
The procedure day schedule:
- 08:00 — Arrival at clinic, final blood tests, anaesthesiologist assessment
- 08:30 — Final standing markings (planned areas photographically documented)
- 09:00 — General anaesthesia or sedation + local (depending on area and volume); sedation may suffice for small single-area cases; general anaesthesia is preferred for medium-to-large-volume and 360° procedures
- 09:30 — Tumescent infiltration: A saline + lidocaine + epinephrine solution is administered evenly across the target areas. This step is critical for pain control, haemostasis and fat loosening — the "tumescent technique" is the cornerstone of modern liposuction safety.
- 10:00 — If VASER is selected, pre-emulsification with the ultrasound probe; followed by cannula aspiration
- 11:00–13:00 — Area-by-area aspiration, symmetry checks, smoothing of borders
Procedure duration: single area (e.g., chin only) approximately 1 hour; abdomen + waist 2–3 hours; 360° contouring 3–4 hours; BBL combination adds a further 1–1.5 hours.
At the end of the procedure, skin entry points are left open or closed with very small sutures. Drainage of tumescent fluid through the 3–5 mm incisions over the following days is expected and desirable. A compression garment (specialist garment) is fitted immediately — it is the most important tool in your recovery.
Recovery — 48 Hours, 2 Weeks, 1 Month, 3 Months, 6 Months
Recovery from liposuction varies depending on the surgical volume and number of areas treated. Typical timeline:
- First 48 hours: Mild-to-moderate pain, a burning sensation and visible bruising are normal. These are managed with prescribed analgesics. Tumescent fluid drainage continues for 24–48 hours — this is expected and desirable (excess fluid leaving the tissue).
- Days 3–7: Return to light daily activity. The compression garment is worn day and night (24 hours for the first 4–6 weeks, daytime only for the subsequent 2–4 weeks).
- Days 10–14: Follow-up appointment, removal of small sutures. The majority of bruising fades. Return to desk-based work is possible.
- Weeks 3–4: Gradual return to strenuous exercise (with surgeon approval). Lymphatic drainage massage can begin during this period and supports swelling resolution and contour definition.
- Weeks 6–8: Most of the swelling has resolved and a clear difference is visible in clothing.
- Month 3: Results are noticeably settling; the new contour is largely visible.
- Month 6: Final result. Complete tissue remodelling, skin retraction and contour definition are assessed at this point. Evaluating the outcome before this stage is premature.

Compression Garment — The Invisible Surgeon
The compression garment is a third factor that is at least as critical to the outcome as the surgery itself. It facilitates the adhesion of skin to the new contour, guides swelling resolution and ensures correct adherence between the skin and underlying tissue. Usage rules:
- First 4–6 weeks: 24 hours a day (except when bathing)
- Subsequent 2–4 weeks: daytime only
- Sizing is adjusted to fit the body precisely — a "loose" compression garment is ineffective
- Silicone-supported or patch-technique garments provide additional benefit in certain areas
Where this rule is not followed, contour quality and the rate of swelling resolution are significantly reduced. The clinic provides the first garment; a backup plan is also discussed.
Who Is a Suitable Candidate? Who Should Proceed With Caution?
Liposuction has a clearly defined target group. It is not the solution for every patient who wishes to lose weight. Candidacy assessment accounts for half the outcome.
Suitable Candidates
- Stable BMI (typically 18–30): Patients whose weight has stabilised and who have a lifestyle that allows them to maintain that weight after surgery
- Localised fat concerns: Stubborn, localised fat deposits that do not respond to diet and exercise (lower abdomen, flanks, back, inner thighs, inner knees, chin, lower back, and similar areas)
- Adequate skin elasticity: Sufficient skin recoil capacity once fat is removed; younger to middle-aged patients with minimal striae and limited sun damage tend to be more suitable
- Good general health: Individuals without uncontrolled chronic conditions who are not taking anticoagulants
- Realistic expectations: Liposuction does not cause weight loss — it creates contour. Patients who have accepted this framework
- Non-smokers, or those who can stop smoking at least four weeks before surgery
Situations Requiring Caution or Postponement
- Significant excess weight / obesity: In patients with a BMI above 30–35, liposuction is not recommended as a weight-reduction tool. The priority is sustained weight loss first (through nutrition, exercise and bariatric assessment where appropriate). Liposuction does not replace the weight-loss process — it follows it.
- Significant skin laxity and striae (stretch marks): In cases such as post-pregnancy abdominal laxity or loose skin following substantial weight loss, liposuction alone does not deliver adequate results. For this group, abdominoplasty (tummy tuck) or a combination plan (liposuction + mini abdominoplasty) is discussed.
- Uncontrolled diabetes: Risk of impaired wound healing; internal medicine control must be established first.
- Autoimmune conditions or significant coagulopathy: An individual risk-benefit assessment is carried out.
- Active pregnancy or breastfeeding: Surgery is postponed.
- Planned significant weight change in the short term: Weight gain or rapid loss after liposuction can compromise the result; surgery is planned once a stable weight has been maintained.
- Suspected body dysmorphic disorder (BDD): Psychiatric support takes priority; surgery alone is not a solution.
- Under 18: Aesthetic liposuction is not performed.
A patient we tell "this is not right for you" is more valuable to us than one we say "yes" to — because an unsuitable outcome benefits nobody.
